Initial areas of the form?

    Hi,

    Are you looking for something like this:

    http://www.jotform.com/?formID=42506564377157

    If so, all you need to do is add a Text Box" that validates for alphabetical characters only and allows for only up to 2 or 3 characters to be entered.  Here is a guide on how you can clone the form to see how it is set up:

    That is exactly what I am looking for but I can't figure out how to create
    it. I don't need to copy the entire form as I have mine set but how can I
    insert it with the "Please Initial" above and the initial box size as shown?

    Hi,

    To create the box all you need is to add a "Text Box".  Here are the setting used to create the one in the form referenced:

    It also needs to be shrunk:

    This should give you the box you are looking for.
