Using dynamic matrix / issue with save and add row button

Hi

Trying to figure out where things are going wrong. I have a three column / header input form and a two column / header input form. You can see from the snapshot that when I save and add a row on the three header section, I do not get the option to edit and delete the entry. However, when I do the same on the two header input form, I get the options to edit or delete the entry. 

Is this a recent limitation of the dynamic matrix form, because it has been working for the last 3 months.

Any help here would be GREATLY appreciated.

for your question Is this a recent limitation of the dynamic matrix form?no there is no limitation the field should work 

I have tested the widget with three column and I am able to delete and edit my entries as shown in the screenshot below

The edit and delete works when an entry as been submitted
