So, students typically select a single class from a combo box.   This class is held at a specific location and at a specific time during a month.   The goal is to get a single form to save or print out with the day date time location city state zip (which I usually put in on combo box (I know it's kind of long that way) but I'm open to anything that will still fit nicely in a mobile format.

    Any help would be appreciated.

    Hi,

Can you please tell us how many classes there would be? Also if they are changed often?

I have created a quick sample of what might be done which you can see here, but once we know more we can help you find the best layout.

If you like the idea and would like to improve it you can do that by cloning the form above. You can see how, here: How to Clone an Existing Form from a URL

    Best Regards,
    Ben
