Do you need to add a email field and file upload field to your JotForm embedded in Adobe muse? You can just edit your form and add the form fields. Once you save your form, it should automatically display these fields in your embedded form. If you do not see these form, you will have to embed your form again in your Adobe muse. Please be noted that you will have to sign in to your JotForm account so that you can edit your form. What is your account username? I am unable to find any details with your email address "firstname.lastname@example.org". You can also let us know your web page URL where you have embedded your form and we will be able to find out your account.
