Yes it is possible to create similar forms. 

There are multiple ways to achieve your requirement. You can add "Payment question" / "Purchase Order" question in your form or you can add form fields manually and then add "Form Calculation" widget to do the calculation.

This seems to be a different form.

What about the form which i created earlier? You do not need that? 

I did check the new form URL you have shared and it is displaying the "Price Estimator" in the same page, next to the form fields. Can we do it at the bottom of the page or create a multi page form to achieve similar requirement?
