I like this option because you can for example put logo in your form of an bank/product and fill the right logo by passing the source as parameter. Example: You can put plain text element above your form with the text: "product: [product_name]" In the url you say ...?product_name=product 1. When you look at the form then it will say "product: product 1". Or with html tags

Have you tried the Pre-populate app? It sounds close to what you're looking for.

You can't create fields based upon it, but you can fill in values. It's possible to set up a form with conditions, such that if field A=1 then show field 1. If field A=2 then show field 2. Thus it could effectively create a form that has different options and values based upon a query string.
Let us know if that will work for your needs. I don't anticipate us allowing users to create fields directly from the URL. It's not a highly requested feature, and allowable URL lengths would limit its utility.
