I checked the notification for your form with the most submissions and it appears your notification has been setup correctly. When I checked our mail logs however, I was unable to find anything sent to your Yahoo email address. We have had issues in the past with Yahoo blocking or filtering email from our servers for some users. If possible, try using a different email address as the only one in your notification and see if the notification is sent on completion of the submission.
