what is the procedure involve this app to be included in app store and how to set up receiving payments

Thanks for contacting Jotform Support!
Right now, we don't have the option to publish Jotform Apps created with Jotform on any app store like Google Play Store or Apple App Store. The Jotform app can only be added to your device's homescreen so you can access it easily and/or shared with others.
