i requested NOT to be charged for the upgraded acct. a month ago

    I WAS charged & have not received a refund.

    I see your last charge was made on Aug 30th, and you requested to cancel the subscription on that date( http://www.jotform.com/answers/422598-Cancel-Subscription-do-not-renew-for-additional-month), but you did not request to have that charge refunded. 

    Upon your request, I have refunded your last charge. If you have another question non-related to this one, please open a new thread, we will be glad to assist you.
