I upgraded my free account to the Premium one yesterday (Order Reference Number: 61743116) 13 Sept 11.
The account is still showing as the free account and I would like to know why the change hasn't taken place. Not a great start!

You are not supposed to enter your e-mail address in the username field when upgrading your account. That is the reason why your account failed to upgrade - your subscription was attached to your e-mail address as the account username, which, in fact, does not exist.
I have cancelled the subscription and refunded the amount. The refund should reflect on your credit card in about a week or two. Please upgrade your account again. This time, enter lfelgate as the account username.
