My name keeps auto-filling on my forms

hi there, i am trying to create a form on my IPAD to set up in public, but everything i test the form, it keeps auto filling my own name in there. I have used the placeholder feature, but that still does not fix it.

I also checked the settings on the IPAD, but autofill is off.

Thank you for your patience. Have you tried to log out of your account and then try to access your form as a guest user? Your name is auto-filled because you are viewing the form while logged in to your Jotform account. It doesn't mean that the other users will be seeing your name too. The form users' names will also appear in the name field when they are also logged in to Jotform. For guest users (not logged in), the name field will remain blank.
