Form not found and Form disabled

Hello erenyilmaz,

Upon checking your account I found that it's suspended by our anti-phishing system. Both your forms "Üyelik Formu" and "Clash of Clans Üyelik Sistemi" has fields like "Username & Password" which makes it suspected of being used for phishing.

Authentication form can't be used in JotForm and creating such forms are against the Terms of Use. These forms must be deleted before we can activate your account again.
