Smart possibility when asking for address details?

Is it possible that a contact form change questions depending on the details you choose in a dropdown menu?

For example if a client can choose from his own address or hotel, if he chooses his own adres the questions are his address and zipcode. But if he chooses hotel then the questions are instead of the adres: the hotel name: and instead of the zipcode the question will be the room number.
