Trying to Create "Conditions" - Form Not Showing Up all on Webpage

    Hi There,

     

    First of all, thanks so much for everything you guys do! My company really appreciates your service.

    I am trying to set up conditions to only reveal a few fields of my form at a time. 
    In your "preview mode" the forms load up perfectly according to the actions taken.

    However, on my WordPress website where I have the form loaded up, NOTHING shows up except for the title at the top and the Header field.

    I have tested it on:
    Chrome (OS X) and Safari (OS X) with no luck.

    NOTE: For some reason, the actions DO show up in Opera.

    Please let me know if you can take a look at it for me.
    I have disabled the conditions for now, but they are still saved in my form titled: "BizCard Design Order Form"
