How do I embed a form into a Google Site?

    I am trying to embed this form into a Google site. I chose embed gadget but it doesn't pull up.

    Can you please try the embedding once again? I have just tried it, and the form is loading fine on my test Google Site page.

    Follow instructions from the Embed Form Wizard.
