The "Choose File" or 'Browse" button is not working in three different browsers (IE, Firefox and Chrome). It seems it is "grayed out" so I cannot even click to look for the file.
I have searched the forums but cannot find anything about this issue.
This is the form I am trying to add a file to: http://form.jotformpro.com/form/42404311847954
I have noticed some forum posts where your support team says you cannot add a document, but rather just link to it. The problem is, my document is a Word document that is hosted online and so when I click on it I do not get a link. it just automatically downloads to my computer.

Hi floydpr,
The file upload is working correctly in the form editor. Although, it is really grayed out when you are in the editor itself. But when you click preview or open the actual form, users can click it and will be to upload files.
If you are trying to insert a file inside the form then this guide might help: http://www.jotform.com/help/153-How-to-Display-a-PDF-Document-On-Your-Form
To get the link of the Word document, you need to Right Click the Link (depending on the browser) there should be Copy Link or Copy Link Address.
Hope this helps. Thank you.

floydprReplied on October 15, 2014 at 11:21 AMOh. That makes sense. I was confused about the function of the field. I wanted the user to click on something that would give them the form that they could download, fill out and print. So, the solution you provided below will not work either. I think I just need to convert it to a PDF so I have a link and can link to that in a text field.
Thank you.

Hi Mark,
I would like to apologize right from the start as I am not quite sure what you would like to accomplish, but based on what I gathered will try to help you.
If you would like to allow them to download the PDF form that they have filled out you can do that by including {pdf-link} in your form's auto responder or email notification.
This will give them a link to the PDF file of their own submission.
If you would like to show a PDF form on your jotform so that your users can see, edit and print out, then upload it to your jotform this widget might be what you are looking for:
http://widgets.jotform.com/widget/pdf_embedder
http://widgets.jotform.com/widget/pdf_previews
Do let us know if any or neither of the two suggestions are what you wanted to accomplish and we will see what we can do to further help you accomplish what you are after.
Best Regards,
Ben
