My form works perfectly in preview mode but when i embed the form on my website all the "CONDITIONAL FIELDS" are showing.
What is causing this?

Are you still having problems with your jotform not working and displaying conditional fields?
I checked your website and it appears to be working on my end.
Can you let us know if this is still an issue?
If so please try clearing your browser cache then check again and let us know if that doesn't help.

Sorry I was trying to use another basic form at the time when you took that snapshot. This is the problem I am experiencing. SEE BELOW
Its showing multiple services even before the condition is selected in drop down form

I took a look at your page and I see the issue. It seems that the issue is being caused by the script conflicts on your page, making the scripts in the js form embed code to not work properly.
What you should do is embed the jotform using this code instead:
<iframe frameborder="0" scrolling="no" style="width: 100%; border: medium none; height: 1384px;" id="42854901884161" name="42854901884161" allowtransparency="true" src="//form.jotform.us/form/42854901884161" onload="window.parent.scrollTo(0,0)"></iframe>
That is iFrame embed code.
