How to Remove Check Box Options (Items)

I have gone in to each check box and removed the check mark and the options. I have done this several times but the form keeps auto filling the boxes. Also, when I try to convert to a PDF it shows all of the options that I have deleted. How can I fix this?

In order to remove the check box options, you will need to click on the field to select it, then use an Options icon on the field Properties tab. Please see the following animation:

You will also need to save the form once you edit it.
