I need help please
After create my form my account not work when i login
Username or password did not match

I took a look at your account and I see that there are 3 accounts. One called faculty.arts632 and 2 guest accounts associated with the same email.
What I would suggest is to try to login again, but this time with your account name (faculty.arts632) and your password.
If this does not work for you, please try to reset your password by going to our login page ( http://www.jotform.com/login ) and clicking on Forgot Password option.
Once you click on the forgot password, please use your username (faculty.arts632) instead of email and click on Send Password Reset instructions.
After this please take a look at your email and follow the instructions received.
If you are not able to login with your username nor reset the password, please let us know and we will be happy to help.

I created my account a few days ago, my username is GeraldineAmat and my email firstname.lastname@example.org
When i try to log in, i have the message Username or password did not match
I tried to reset my password, but i still have the same message.
