Blue Envelope contact form lost style

Hi,

Received support earlier today to tweak a few errors on this form.

Have just been onto my side and the whole envelope section is distorted with the "mail it" tab being far too large.

I have checked the form in the designer area of jotform, where all appears to be find.

any assistance would be greatly appreaciated

I checked and the standalone version of the jotform is displaying correctly so this must be a conflict between the jotform and the website codes:

You may try embedding the jotform again, using the iFrame method preferentially as it is a "conflict free" option to embed the form.
