I am not sure if I have understood your question correctly.
Do you mean to say that your form is not being displayed in your web page or you are not able to see your forms in your account?
I did check your account and did not find any issue with that. There are 4 forms in your account. Please check the screenshot below:
Are you not able to see these forms in your account? I would suggest you to clear your browser cache and see if that solves your problem. The following guide should help you in clearing browser cache: http://www.wikihow.com/Clear-Your-Browser's-Cache
