    Hello,

Are you referring to the form embedded on this page: http://www.lipoedema.co.uk/support-us/membership/?

If this is correct, I'm sorry but the form on that page is not being handled using Jotform.
The URL of the form is: http://forms.proximity-hosting.co.uk/form/22753051633

It seems like they used the full source code of a form created in JotForm and then modified it to use their own domain (proximity-solutions.co.uk) which means that you cannot edit it this form using JotForm.

However, you can clone it using this URL: http://forms.proximity-hosting.co.uk/form/22753051633 and then replace it in your site.

Please let us know if you have further questions.

    Thanks.
