Today i got email from jotform team that.
Our automatic phishing detection system has suspended your JotForm account. Your forms are currently disabled.
If you would like us to review this decision please reply to this email and provide these information:Describe the purpose of your forms. If you have asked for username or password in one of your forms, please describe the reason. Provide the URL of your web site where you use these forms.
We will try to resolve this issue as quickly as possible.
I replied on your email could you please check. The message which i replied back is1. We are using forms for just contact information.2. We never asked username and password in any form you can check all form which we have created.Page URL:

Upon checking your account I found that it's suspended by our anti-phishing system. We have automated anti-phishing system which scans all forms and mark them with probability of being used for phishing activities.
Upon taking a closer look at your form, I can confirm that it's not being used for any phishing activities. I have activated your account now.
