My form no longer shows up on website

For several days I had an embedded Jotform on my Tumblr page; however, several days ago the html coding failed to show up on the site itself. For the moment I've redirected the page to the HTML page version of the form. I would like to ask you for some troubleshooting tips on how to ensure that the embedded form can reappear on my site

There could be some conflicts with the embed code of Jotform and your code on your site. To avoid that please try the iframe code and see if that works. IFrame should eliminate any conflicts between Jotforms code and yours.
