My form completion for PATIENT UPDATE has sub-fields within the same text area.  See below.  I cannot make this happen, but may be using the wrong method.  I've included an excerpt from my form with examples of some of the items that should always appear for the user. Please let me know.

    Hi,

I presume that this is how you would like your jotform to look when someone comes to it: http://form.jotformpro.com/form/43111928373958

Now to do that you should click on the textarea widget and then click on the Default Value button in the Toolbar. This will open a small screen for you where you can type in the default values.

Do let us know how this works for you and if you have any further questions.

    Best Regards,
    Ben
