At both these screens Log in to account and request new password, when I click on the submit button nothing happens. The screens stays the same and I cannot progress.
Trying to upgrade so I can receive more survey replies.

Upon checking your account, I see that you're using a guest account.
You need to register first here: http://www.jotform.com/signup using the same email address: firstname.lastname@example.org and your form will be automatically moved to this new account.
You can register for a FREE account and this will increase your limit to 100 monthly submissions among other benefits.

Having trouble with username. I have tried a number of combinations, just letters, just numbers, combination of numbers and letter but keeps coming up with username invalid.
Can you let me know what is needed for user name.
I have tried kerrie.smith, kerrie smith, kerrie2299, 9319_00
Only alphanumeric and the underscore '_' characters are allowed in the username.
So, from the list you posted: kerrie2299 should work.
