Hello abomohammad,

Upon checking your account I found that it's suspended by our anti-phishing system. As your form "بروس لخدمة الشبكات" has fields like "Username & Password" which makes it suspected of being used for phishing.

Authentication form can't be used in JotForm and creating such forms are against the Terms of Use. This form must be deleted before we can activate your account again.
