Can I create a login form that can retrieve information from a database to provide access to a second form?

Could you please clarify it a bit more?

If you mean that you want to use a login form that would ask user credentials (username/password), unfortunately that is against our terms of use and your form will be blocked automatically by our phishing scanner.

Also, our forms are available for public access and in order to make them private, you'll need to use the full source code of the form and host it on your own website with restricted access. You'll also have to use a web programming language such as: PHP or ASP.NET to run queries in your database to get the information you need.
