¿Is there a way to put a tracking code to one form?

Firts of all, thanks to anyone who colud help me on this.

The scenario is this: i have a form (called form 1) where i ask for the basic info such name, e-mail, etc. Then using my autoresponder, i wanna send a second form (called form 2) where i ask for more info.

So, i want to know who is the person filling the form 2 without asking again their basic qustions asked on form 1... Is it possible to insert a code on the form 2 knowing from wich e-mail this came from?
