Entering form into contest

Hi There,

I designed a form to enter into the web form design contest. I read everything I could find but there doesn't seem to be anything that tells me how to submit my design. Do I email the link in or is there a button I am missing?

Regarding your question, have you sign up using your name and email with the form here http://www.jotform.com/awards/? You should receive instructions on how to submit your work after completing sign up and further instructions on how to proceed.
