Forms not accessible/disabled

    Could you please "unlock" my forms as I need to allow those people within my business group to make requests of projects.

    Hi Glenn,

    Your account was automatically suspended by Jotform's anti Phishing detecting system because of the passcode field that you have created, this is a violation of our Terms, please see this link: under the paragraph "Phishing". To explain, Jotform cannot process authentications, and can only collect data from users who use your forms. Which is why the system suspected your form to be used for phishing.

    I can reactivate your account, but only if you can confirm that you will delete or rename the fields that uses "passcode", "password" or anything that might resemble a log in page.
