Form shows disabled on facebook tab

hello, i'm been trying to have the form i created with your tools to Facebook but i keep having this weird information telling me the form is no where to be found and it's disabled. i have enabled  the form already and i don't know why i keep having this ugly message. please help!

As I check your account, I see two JotForms in it and both are working fine. Since you are receving this error on facebook tab, I suspect that there should be some issue with the embedding procedure. We request you to try re-embedding your JotForm once again by following the guide below.
