The output from my JOT form is not showing up in my Google spreadsheet

Hi,

I have two JOT forms set up to output the submissions to Google spreadsheets. The outputs just stopped showing up on the spreadsheets on my Google drive.

What might be causing this?

There are two different URLs, but your form only allows one.

Here are the two URLs:

First off, we apologize for the inconveniences caused.

Have you tried re-integrating your form to Google Spreadsheet? I would suggest to give it a try, this should help. If it's working fine before and it then suddenly stopped, it could be an update made on the Google side or a problem at some point with Jotform. So far, we don't have similar reports about this at the moment so the system is totally fine. Please try to re-integrate your form to Google Spreadsheet and see if the issue is resolved.

But before doing that, kindly confirm my test submission on this form http://diversitycenter.org/home/submit-your-local-lgbtq-event/ if it's still not forwarded on the integrated spreadsheet.
