Form not found / form has been disabled

    My forms no longer display on my website and when I login to preview my forms I get a form not found and form disabled message?  I'm not sure why, please help me fix them.

    Hi Shane,

    One of your forms is asking for login information such as Google Account, Facebook, and many others. This kind of form is not allowed into our system. We consider that as phishing. Please kindly check our Terms of Use: http://www.jotform.com/terms/ before creating a form.

    Hi,

    Yes sir, you are right.  I didn't read the terms of services.  So I have one form where I need to collect clients login information and I believe that is what locked forms.  Can I continue using your services if I use the forms correctly going forward?

    Can I delete that form and will it make my other forms work again?

    Hi Shane,

    I have reinstated your account and deleted the login fields. You're good to go. Please keep in mind that login fields or username and password combinations are not allowed here. We do not support it. Kindly refrain from creating such form to avoid account suspension or permanent termination. You can now continue working with your forms.
