How can I take my form back to its Original Format?

I notice that fields like the Name and set up, breakdown times are weird looking? I want to use the SOLID FORM TEMPLATE. How can I use that without redoing the entire form?

You can use the form revision history to revert the changes made in your form to a previous state. Or you can also use the form designer to fix the fields that are not looking the way you want in your form.
