Can I pre-populate half of the form with individuals information and then send the forms to the respective individual only?

I want to send a form out to approx 600 people.  I want to pre-populate half of the information (name, department, etc.) while only leaving half of the fields for people to fill out themselves and then submit.  I know how to pre-populate the information using your existing app.  But once I upload the information, can I then send out the personalized form to each individual person?

You meant, you wanted to provide the form URL/link http://www.jotform.us/form/42655531391152 to specific individual right?

You can use the email... how about if you just send an email to the target individual and provide the form URL link in the email message?
