Form is not capturing data in the credit card information section. Please help.

Your jotform on that page: https://secure.jotform.us/form/32978613180156 is capturing credit card information outside of our widget that should be used for that.

You will need to remove these fields or your account will be terminated as per our terms of use.

Once you remove them you can set up the widget built for this purpose. I suggest taking a look at these guides:
