I just created a form but it's not showing up in My Forms

I created a form. Saved, Preview....

I'd like to send out the link to the form but when I go to My Forms and click on the name of the form, the form I created is not there. Something else is in it's place.

If I press the back button, I can find the form I created.

Where is the form I created? Not the one that is there.

This is a 'Fundraising Registration Form' and it is not currently in folder. Please click on the My Forms link at the left hand menu and you will see the form in question.

That form isn’t the form I created. I used it as a template and created additional fields.
