Why am I receiving a FORM DISABLED message?

I receive a FORM DISABLED message when submitting a feedback form from my CONTACT page on my website.

There might have been a small hiccup somewhere for a bit that was preventing submission.  Let us know if your form still comes up as disabled upon submission and we will be happy to have another look.
