I checked your form in Chrome, FireFox and Safari and the signature widget showed up in all 3:
May I ask which browser you are using that has it show up blank? As soon as we can replicate the issue we can start working towards fixing it.

I just deleted the widget and reloaded and it worked fine. I tried to delete this post before anyone responded, but don't know if users are able to do that.
