I can lock the ipad, of course, but the ipad asks for an app on the home screen that will become the default only screen available to anyone trying to use the ipad. I do not want people checking their email or surfing, I only want them to be able to take the survey. Thanks if anyone can help.

Hi Linda,
Unfortunately, this is not possible with the JotForm builder. First, is that because the form is embedded on a website and being viewed in a web browser, the iPad is a separate entity which is the device itself and forcing the user to lock their focus on the survey would not be possible right now. You can, however, give notifications like alert or prompt box that will notify the user that he/she will be leaving the web page, but this by editing the Full Source Code of your form and adding a custom Javascript which needs more programming skills and you will need to embed the form's full source code to the website itself.
I hope this helps or at least gives you an idea on how to proceed. Let us know if you need more information regarding this.
Best regards.

I just thought I’d let you know that it is possible, if someone else has the question in the future. What I did was go the link to the survey, add that to the home screen on the iPad (which makes it an “app”) and then lock (password protected) the iPad so people can’t go anywhere else on the web. It was easy once I thought it through.
