How can use a form more than once on Ipad?

I do driver evaluation on my Ipad.  I will do 2 or 3 of these each day.  How can I place this form on my Ipad so that I can use it multiple times.  I want send each evaluation to my email and to the drivers email.  The form that I developed does exactly what I want it to do.

You can access the forms using its URL on any browser. 

Example, if this are your jotform's URL

http://www.jotform.us/form/40496599507166

http://www.jotform.us/form/40656624959164

You can open each form using your iPad internet browser on different tabs.

When you submit the form/s, it should send the Notification Email to the intended recipient email address.
