    6) I'd like to make another form to get self-reported feedback at visit one, and then have the form be able to 'mine' and display the data from the previous visit when they come back for a reexam in say 12 visits time...display the original data as 'read only' so they can compare and contrast their new responses to their previous ones. Do you have something that could achieve this?

    Hi Ben,

    We don't have a direct feature that would accomplish this, but a workaround would be to create an autoresponder email:

    https://www.jotform.com/help/26-Creating-a-Form-Autoresponder

    In the autoresponder email body, add the following line:

    https://www.jotform.com/submission/{id}

    {id} is the Submission ID tag which will be converted to a string of numbers. This is unique and system generated. Ask your form submitters to keep the link for later comparisons.

    For example: https://www.jotform.com/submission/293951721023769986

    Thanks

    So, rather than an auto-responder...I could have a notification email with
    this sent to me, and then I would be able to populate a new form with this
    data (in a read-only format) as well as having a live version for them to
    add their current responses?

    Yes, you can use the workaround provided by Welvin with any type of email notifications, so it can either be an autoresponder or a notifier sent to your email address.

    If you have further questions, please let us know.
    Thank you.
