Any time we submit a form, it is not forwarded to our designated email. Would this be an issue on your end or would it be with our mail server?

I have checked your jotform and I see that the email used is in our bounce list:
jo**@sta****s.com is IN the bounce list
I have removed it from our bounce list so you should be getting emails again starting with this reply.
In future, you can also test if your email gets into bounce list and remove it from there, by following this guide: How to remove your email address from bounce list
