Why is my form displaying incorrectly?

Hi, Please help! Here is the form I've designed, ready to embed on my website in a 266x172px space:

But when I preview or copy and paste the code, the result looks like this:

This doesn't fit the space I've allowed and looks terrible. What can I do? Please bear in mind that I dont understand any CSS at all.

Please try to embed it to see how the form would look like after. You are on the Test Embed Window and the result might be different when actually embedded.

Sorry, I should have explained that I already tried to embed the form and the result looked the same as the test window shows. I tried copying the code with various methods on the jotform site (embed/iframe etc). but they all give the same result. I tried creating a new blank page on my website with a large sized html snippet box but it was just the same. Do you have any other ideas what might be the problem?
