Disabled account and jotforms

    my form is said to disabled. what to do to make it not disabled

    Hi,

    I took a look at your account and I see that it was disabled because of this jotform:

    This happened because of our anti-phishing system detected some signs that are usually found on phishing forms.

    To avoid this please give a name to your jotform. Also can you please tell us the purpose of username and profile link in it? I do not see a password field on it, but adding one would be disabling your account again.

    Now I have activated your account and whitelisted your jotform. What this means is that it will still be closely monitored by our phishing system, but would not disable your account unless it finds a good reason for it.

    I deleted my form and retrieve it from trash (thanks Lord)

    but, my app received the address of the form (I use an app from "fabrica de formularios" but

    the app receives a message saying ( Document not available), if I insist, a clean document is done with (Principal plate and a button)

    I have the request form in my screen, I tried to save it and then send the link to the other app...but is still continues as "not available" or " Principal plate and a send button"

     

    How can I retrieve the request form, takes loooong time to rebuild that....
