error receiving on submitting the form

estou querendo colocar meu formulário em meu website, mas esta dando um erro, quando coloco o codigo fonte do formulario na pag do site, (pedido de oração e a pag testemunhos) do site entra uma outra pag...o que será esse erro, podem me ajudar...aguardo contato.

I see your two JotForms are embedded correctly on your website and seems to be working fine.

Also, I was able to these two JotForms without any issues. If you are not referring to this and continuing to receive error message please let us know and it would be helpful for us if you can provide us with the screenshot of the error.
