The problem is related to the DDoS attack that was made by Brazil's based IP Addresses. We blocked those IP Addresses and it seems like your IP Address was affected and that's the reason why you're not able to view your form and some of your customers/users.

I have installed JotForm form in several of my sites through a pro signature, it's been more than 24 hours that do not receive any contact form, and usually get more than thirty submissions per day. If my IP and my customers in Brazil will Remain blocked due to DDoS attack then my clients will not be able to contact me.
I will await the normalization of service for a few more hours. I hope this becomes normal, because I was so happy with the JotForm ever wanted to sign a larger plan to use in high-traffic sites.

Your team BANNED THE WHOLE FUCKING BRAZIL
I received 350 posts in the past 3 days, now I will have to search for a new solution.
this is not acceptable. I am losing money because your team doesn't know to stop a DDOS attack.

We have received the confirmation from our back end team that the Brazil IP addresses are now enabled. That should have resolved the issues you were having.
