Page headers are not showing up on the completed form submission

Page headers are not showing up on the completed form submission (either in the email or, on the submissions page. Without these headers, it is difficult to determine which fields were submitted under which section.

Thanks for reaching out to Jotform Support. I tried to check your account to see and test your forms but there is no Jotform account under that email address. If you have an account at Jotform can you reach out to us from there?  It would help us if you can share the form link so that we can try to replicate the issue. This guide will show you where to find the form URL. After we hear back from you, we’ll have a better idea of what’s going on and how to help.
