Hide a few fields when a checkbox is checked

1. So I need one little edit :)

When someone checks the top box to buy test traffic, the two below it would then need to disappear like this--> http://prntscr.com/5mj4cx

AND

When the top box is checked, if you can remove the "clicks x days" as well, that would be nice too.
